Affinity Family Care thanks you for your interest
in our company. We are an agency contracted with the State of Arizona,
Division of Developmental Disabilities to provide in home care services
to individuals with disabilities. The first step in applying with our
company is to fill out an Application for Employment. You
must be 18 years old to apply.
Upon review of your application and if hired by Affinity Family Care below
are the requirements that must be met before being hired:
All of the training listed below is provided at NO CHARGE to you and must be completed prior to starting a position with Affinity Family Care, LLC.
Article 9 Training (State Mandated)
CPR Training
First Aid Training
Habilitation and Positive Teaching Strategies Training
Data & Documentation Training
In addition to the training, the following requirements must be met:
Original Application for Employment
Original (3) References on Reference Request DDD Form
Employee Emergency Contact Form
Copy of Drivers License
Copy of Social Security Card
Finger Print Clearance Card OR Application completed and fingerprints
taken
Criminal Record Self-Disclosure Form NOTARIZED
Transportation Release Agreement
Employee Drug & Alcohol Test Agreement
Our providers are not just sub-contractors, but are empowered employees.
As employees of Affinity Family Care we tax your paychecks. As a result,
the following forms must be completed:
I-9 Employment Eligibility Verification Form
W-4 Federal Witholdings Form
A-4 State Witholdings Form
Although your employment with Affinity is At-Will which means you may
cancel your employment at anytime we require that you:
Sign and Date an Employee Agreement and Acknowledgement of Orientation
& Policies, which is explained in training.
